Super lush sounding Stereo Chorus from Guyatone. Made in Japan in the 1980s. Everything works great and the pedal is in nice shape for its age. 

Shows some cosmetic wear as pictured. Includes pedal only, no cables or other accessories. 

It has separate controls for its dry/effect blend for the A and B outputs giving you full control over the stereo chorus effect.
